Sick and can’t work? Please follow these steps to report your sick-leave online:

  • Step 1: Call your job the 1st day you are sick and inform them. If you are sick and need medical attention, contact your family doctor for assistance.
  • Step 2: Report to SZV online: click HERE
  • Step 3: The SZV control doctor will call you the same day that you fill out the online form, don’t miss the call!
  • Step 4: SZV will e-mail you about your approved sick-days. You must give this e-mail to your manager/HR.

The procedure to renew or apply for SZV medical insurance is digital.

If you are 18+ years and ZV insured or 62+ insured:

Click HERE to request your personal account for the MySZV portal. In your MySZV account, you can check if your medical insurance status is valid or not valid.

  • With a valid medical insurance status, you will only need to show a valid Sint Maarten ID card or passport to your health care provider to get medical care or medication.
  • If your medical insurance status is not valid, please contact SZV using the webform below.

In the following situations, please use our webform below:

  • Your medical insurance status is not valid, in your MySZV account
  • First time applicant ZV (you are an employee or a dependent)
  • First time applicant 62+ / FZOG
  • Renewal FZOG

Report online for maternity leave up to 2 months before your expected due date.

  • Step 1: Get a signed letter from your gynecologist or midwife, indicating your due date. Make a scan copy of this.
  • Step 2: Report to SZV online: click HERE.
  • Step 3: E-mail a scanned copy of the signed letter from your gynecologist or midwife stating your due date to sickleave@szv.sx.
  • Step 4: SZV will e-mail you about your approved maternity days. You must give this e-mail to your manager/HR.

Baby Guarantee Letter: Your request for a baby guarantee letter cannot be submitted before your 7th month of pregnancy.

  • At least one of the parents must have residency to obtain a baby guarantee letter.
  • If the breadwinner is not ZV insured you will not qualify to receive the baby declaration.

New Born Registration: You must register your newborn within 10 working days by SZV.

  • The newborn can be registered as a dependent of the head insured.
  • You will need to e-mail a scanned copy of the detailed census registration form and birth certificate to insured@szv.sx.
  • If additional information is needed, SZV will contact you.

We advise to apply 6 months in advance of reaching pensionable age as this is a lengthy process.

When can you apply for AOV Pension:

  • All who were born before 1958 can apply at 60 years.
  • All who were born in 1958, 1959 and in 1960 until and including June 30th can apply at 62 years.
  • All who were born as of July 1st 1960 and up can apply at 65 years.

How to apply:

Submit the filled in application form and the required documents via e-mail to benefits@szv.sx. These can be found HERE.

After applying SZV will contact you within 3 to 6 months if there are any questions about your application.

If there are no questions, your decision letter will be sent to your mailing address.

Patients DO NOT come to SZV for their approvals/stamps for diabetic strips, lancets or medical aids.

The treating physician will write a prescription for the medication, supplements or medical aids and provide this to the necessary provider (pharmacy, medicosmetics) who will email SZV for authorization.

Approvals will be given for 1 month.
